"Based on the Original Scenic and Costume Designs by" - There is otherwise no Costume Design credits, only wardrobe and dresser credits
"Based on the Original Sound Design by" - There is otherwise no Sound Design credit
"Based on the Original Hair and Make Up Design by" - There is no other Make Up Design credit, but there is a Make Up Artist credit
The film is Barrymore, 2011, starring Christopher Plummer, and is adapted from the stage play.
