I have Louis Lazzara (2 z's) also in Friday the 13th 5 (credited with one z in that), right under David B. Miller, both for make-up effects. He's also in Freddy's Dead right under David B. David for make-up effects, Louis for make-up artist, and Wild at Heart under David. I don't believe they are the same person due to this. FWIW, I have David B. as the common, here's what I have for all these names, sorry for any doubles with previous confirmations.
