No idea, most likely a quirk in the system.  But your covers were approved so in the end, it's all good.

I more or less asked the same question in this thread after which I sent a PM to Ken and got this reply:

But does it make sense to decline something what does not exist? 

E.g. I can see in my contribution list that some images and profiles have the status "Released", although I only contributed the images. In this case it would be better to leave the profile status empty, otherwise I do not understand the the intention of the separate columns?! Or did I miss something?

when you Refresh the online and then Refresh Updated Profiles the cover changes are crossed so you see there is a cover change that you can preview.

Yes, I'm still having this happen to about 10% of my "Image-only" contributions, several happening just today.

I might as well list them, since users will most likely otherwise not know that the images were updated.

I've just had one of these as well.
Profile declined but image accepted even though I only submitted images.
