I wanted to vote other but could not. I'd enter the CoO only to those box sets, where it is the same for all the main features. For box sets with mixed CoO, I'd leave the field empty. That's the approach I take for all the fields  in a box set profile where the rules do not spell out something different.

This strikes me as leading ultimately to mass confusion. There is ONE primary production company for every film  and that production companies location determines where the film originates. For example, The Never-Ending Story was a German production and US distribution was handled by Warner Bros. Multiple possibilities leads to location issues, let's see Thunderball was filmed in the US, the Bahamas and England, I seem to recall a French locale as well. With that in mind, i think the best answer is the free edit field, such as we use for Edition, if the Country is not listed you can add it to the list. The checkbox with multiples thereof seems way to much of a nightmare scenario.

Filming location is not related to CoO.

It may not always be possible for us to dtermine which is the primary production company.

For more than 90% it's no problem, there we have, as you say, only ONE primary production company.

No, there are films which are co-productions of more than one production company. And determining the leading or primary company can be difficult or even impossible. And yes, this has nothing to do with neither distribution companies nor filming locations.

I do not understand why you can not vote, because as I read you correctly you could have chosen the first option.

Then I do not understand your first option.

If all the features are from the same CoO, I see no harm in including it for the box as well, otherwise leave it empty imho
